Thrombus burden quantification: an evolving tool for coronary interventions

چکیده
Thrombus is a distinctive histo-pathological feature of patients presenting with STelevation myocardial infarction (STEMI) and constitutes an adverse finding that significantly affects interventional management. Characterization and quantification of coronary thrombi have been a challenge since the beginning of thrombolysis era. Almost 20 years ago, the Thrombolysis in Myocardial Infarction (TIMI) group developed an angiographic thrombus scoring system, relating the maximum thrombus length to the reference vessel diameter. These investigators described 6 thrombus grades (G0 to G5), assigning grade G5 to complete total occlusions [1]. Once the safety and effectiveness of coronary stenting in thrombus containing lesions was proven [2], several investigations consistently demonstrated an association between thrombus burden (TB) and tissue perfusion parameters. However, most of these early studies lacked statistical power to evaluate late clinical outcomes [3].

Anatomic characteristics and clinical implications of angiographic coronary thrombus: insights from a patient-level pooled analysis of SYNTAX, RESOLUTE, and LEADERS Trials.
BACKGROUND The distribution of thrombus-containing lesions (TCLs) in an all-comer population admitted with a heterogeneous clinical presentation (stable, ustable angina, or an acute coronary syndrome) and treated with percutaneous coronary intervention is yet unclear, and the long-term prognostic implications are still disputed. This study sought to assess the distribution and prognostic implic...

AIMS Manual thrombectomy has been proposed as a strategy to reduce thrombus burden during primary percutaneous coronary intervention (PCI) in patients with ST-segment elevation myocardial infarction (STEMI). However, the effectiveness of manual thrombectomy in reducing thrombus burden is uncertain. In this substudy of the TOTAL (ThrOmbecTomy versus PCI ALone) trial, we compared the thrombus bur...

Intravascular ultrasound (IVUS) has been an important modality in understanding the in vivo pathophysiology of coronary artery disease and in predicting outcome of percutaneous coronary intervention (PCI). The IVUS features associated with acute coronary syndromes include positive remodeling, large plaque burden, ruptured plaque, and intracoronary thrombus. Plaque burden, lesion site calcium, a...
متن کاملEffect of thrombus aspiration on angiography and outcome in patients undergoing primary coronary angioplasty.
Thrombus exerts a major impact on the performance and outcome of primary and rescue interventions in acute ST-elevation myocardial infarction. Although the optimal treatment of thrombotic lesions is still controversial, thrombus aspiration provides an effective method to achieve successful reperfusion during primary angioplasty. We compared clinical and angiographic outcomes in 286 patients wit...

RATIONALE Coronary artery ectasia (CAE) is characterized by an abnormal dilatation of the coronary arteries. CAE is often associated with the presence of slow coronary flow and may lead to acute myocardial infarction (AMI), even without total occlusion. PATIENT CONCERNS AND DIAGNOSIS We report a case of a 24-year-old male patient with CAE suffering from AMI. INTERVENTIONS Percutaneous coron...
